OADC Policies
Since OADC always strives for excellence in patient care and safety, certain policies are in place to ensure comprehensive standards for cooperation.
A short summary of the major policies is listed below. These deal with registration, payment, cancellation of surgical appointment, refunds, surgical procedure and patient satisfaction.

Thank you for allowing the OADC team to service your anesthesiology care.
+ Registration - all forms must be received by OADC at least two weeks prior to surgery.
+ Payment - initial deposit, to hold the appointment time, is required two weeks prior to surgical date. Any balance for services rendered will be due on the surgical date.
+ Cancellation - OADC must be notified of any cancellations due to illness. Call the office at 630-620-9199 or

+ Refunds - in order to obtain a full refund, OADC must be notified two weeks prior to surgical date, that you would like to cancel the appointment. Penalties apply if this one week notice is not given.
+ Surgical Procedure - all forms must be presented to OADC staff prior to surgery. All pre-anesthesia instructions must be followed. Post-anesthesia instructions will be given on the day of the procedure.
+ Satisfaction Survey - after your surgery, please assist us in assessing and improving our services by taking just a few minutes to complete our satisfaction survey.
